I. Issuance of Birth Certificate Under the Program of Registration of Vital Events

        (a) Essence. – It is mandated by the Civil Registration Law (Act No. 3753) that registration of vital events is compulsory, for it plays a vital role in the life of every individual.  The Office of the Municipal Civil Registrar shall therefore register specific vital event as the Registration of Live Births within the reglamentary period of 30 days as mandated by law.

        (b) Processing Time. -  If the requirements are complete, this transaction shall be completed within forty-five (45) minutes; while registration beyond the 30 day reglamentary period or late registration of live births shall be completed after 10 calendar days and forty-five (45) minutes.

        (c)Clientele. – All citizens of the Philippines, particularly the residents of this Municipality.

        (d) Requirements:

1. For Registration of Live Births within the 30-day reglamentary period:

        1.1. Complete data about the individual/child to be registered as the:

                        a. Name of the child

                        b. Date of birth

                        c. Place of birth

                        d. Gender

                        e. Birth order

                        f. Time of birth

                        g. Complete name of child’s parents

                        h. Complete name of child’s parents

                        i. Age of parents at the time of birth of the child

                        j. Residence of mother at the time of birth of the child

                        k. Name and address of attendant

        1.2. For Registration of births beyond 30 day reglamentary period or late registration:

                        a. All the necessary data for the on-time registration of birth and Community Tax Certificate.

                        b. Affidavit of two (2) disinterested persons having knowledge of the event;

                        c. Any two (2)  of the following supporting documentary evidence:

  • Medical record of the child if more than one (1) month old and above
  • School record if the child is of school age
  • Baptismal Certificate if the child was baptized
  • Voter’s Registration Record if 18 years old and above
  • Marriage Certificate if the registrant is married
  • Community Tax Certificate

                (e) Availability of Service. – During office days:  Monday to Friday from 8:00 A.M. to 5:00 P.M.  NO NOON BREAK.

                (f) Fees and Charges. -  The acknowledgment fee and service fee shall be based on the rates prescribed in the existing municipal tax ordinances.

II. Application for Marriage License

(a) Essence. - It is the policy of the government that all those who would go into marriage shall have to apply first for issuance of the Marriage License. It shall therefore be the task of the Office of the Municipal Civil Registrar to serve those applying for marriage license.

(b) Processing Time. - If the requirements are complete, the transaction shall be completed after ten (10) calendar days and the marriage license can be available at the same time.

(c) Clientele. – All  citizens of the Philippines, particularly the residents of the Municipality.

(d) Requirements:

1.   For 18 years old and above but below 21 years old:

       (a) Proof of age, any of the following:  (Certificate of Live Birth, Baptismal Certificate, Voter’s                                           Registration Record, School Record);

       (b) Certificate of Marriage Counselling;

      (c)  Parental consent

2.  For 21 years old and above but below 25 years old:

     (a)Proof of age;

     (b) Parental advice;

     (c) Certificate of Marriage Counseling;

 

3.    For 25 years old and above:

        (a)  Proof of age;

        (b)  Certificate of Marriage Counseling;

4.    In case the applicant has been previously married:

        (a) Certificate of death of the deceased spouse, or

        (b) Judicial decree of annulment or declaration of nullity of previous marriage           

 5.    In case the applicant is a foreigner:

(a)  When the foreigner’s country has an embassy or consulate Office in the Philippines; Certificate of legal Capacity to Contract Marriage to be issued by his diplomatic official;

(b)  When the foreigner’s country has no embassy or consulate in the Philippines; Affidavit of Legal Capacity to Contract Marriage, and a certification that he has no embassy or consulate in the Philippines to be obtained from the Department of Foreign Affairs.

6. Posting of applications for a marriage license for ten (10) consecutive days before the LCR issues the license;

7. Suspension of issuance of marriage license for ninety (90) days when the applicants fail to comply

     with requirements of parental advice and certificate of marriage counseling

8. Marriage License can be secured in the city or municipality where one or both of the parties to      

     the intended marriage habitually reside;

9. Optional requirements:

   (a)  Police Clearance for every applicant for a marriage license;

   (b)  Certificate of No Marriage (CENOMAR) for every applicant for a marriage license.   

        (e)Availability of Service: During office days from Monday to Friday and from 8:00 a.m. to 12:00 noon and 1:00 to 5:00 p.m.

        (f) Fees and Charges. The acknowledgement fee for unmarried parents of the child, Service Fee for Application for a Marriage License, Posting Fee and Marriage License Fee shall be based on the rates imposed under existing municipal tax ordinances.

III. Issuance of Death Certificate Under the Registration of Death Programs

(a) Essence. The government keeps and maintains records of deaths for it to be aware of the decrease in population in terms of death and the reasons for death. Vital events, like death, shall be reported to and registered by the Office of the Municipal Civil Registrar.  

(b)  Processing Time. To register vital events as the Registration of Deaths within the 30 day reglamentary period, the transaction can normally be completed in 1 hour and 25 minutes, depending on the availability of the MHO during the registration. On the registration of deaths beyond the 30-day reglamentary period, the transaction shall be completed after 10 calendar days.

(c)   Clientele. All citizens of the Philippines, particularly the residents of this municipality.

(d)  Requirements:

(1)  On Time Registration:

  1. Barangay Chairman’s Certification stating the occurrence of the events in the barangay.
  2. Data needed for the registration:

i.      Complete name of the deceased;

ii.     Date of death;

iii.    Place of death;

iv.   Age of the deceased;

v.    Residence of the deceased;

vi.   Civil Status of the deceased;

vii.  Religion of the deceased;

viii. Place of burial or crematory.

 

(2)  Beyond the reglamentary period or delayed registration of deaths:

(a)  All data and requirements for on time registration;

(b)  Affidavit of two disinterested persons having knowledge of the event.

(c) Any documentary evidence to show that death occurs in the place

(e) Availability of Service: During office days from Monday to Friday and from 8:00 A.M.  to 5:00 P.M.  NO NOON BREAK.

(f)   Fees and Charges. The service fees for on time registration of deaths, delayed registration of deaths, and for burial permit are defined in the existing municipal tax ordinances. The burial permit is P20.00.
